The following documents must be presented at the time of Application for Admission:
Completed registration from and certified birth certificate - copy to be kept by the school
Baptismal certificate - Catholics only
Immunization records – MS Form 121 from Health Department or physician
Copy of official transcript, final report card, test scores and medical information from last school attended
In the event of court ordered custody, the custodial parent/guardian will furnish the school with a copy of the court order establishing custody.
Accident Insurance - one of the following is required:
A. Proof of adequate student accident insurance (copy of insurance card).
B. A statement signed by the parent certifying that insurance covering personal injury of the student is maintained by the family. This information will become part of the student’s emergency file.
